The scheduling in the Pulmonary and Critical Care Fellowship program at Upstate Medical University is meticulously designed to provide fellows with a comprehensive blend of clinical and research opportunities. With a focus on meeting the requirements set by ACGME and ABIM, the program ensures that fellows gain significant patient care experience while also offering elective courses to further enhance their knowledge and skills in the field of pulmonary and critical care medicine.
The requirements the ACGME and ABIM placed on a fellowship in terms of patient care experience are significant. At a minimum, they must complete a three-year program with a minimum of 18 months of direct patient care. In reality, the board requires 18 months of patient care experience in each discipline for a total of 24 months but gives double credit to 6 months of time where the 2 disciplines are closely interwoven. Of these 18 months therefore, 9 months must be devoted to pulmonary, 9 months in critical care medicine, of which 6 months are devoted to the care of critically ill medical patients (MICU/CICU or equivalent); at least 3 months devoted to the care of critically ill non-medical patients (SICU, burn unit, transplant unit, neurointensive care unit or equivalent).
This experience should consist of at least one month of direct patient care activity with the remainder being fulfilled with either consultative activities or with direct care of such patients. OB ICU, NeuroICU and SICU will be mandatory. For neuroICU, fellows will have direct patient care responsibility.

National Jewish Health Elective
At National Jewish Health a large number of patients are seen with nontuberculous mycobacterial (NTM) or "atypical" infections, including M. Avium complex, M. Chelonae, M. Abscessus, M. Simiae, M. Kansasii and other species. Most of the patients have pulmonary disease, while others have extrapulmonary infections, which include spinal, osseous, or soft-tissue sites. We also see patients with MDR-TB, an almost exclusively pulmonary disease. These patients are treated on the ward/adult-day unit (ADU) and/or are seen in the Outpatient Clinic.
Fellows will work with one attending physician, but will regularly interact with the faculty of the Infectious Diseases Division (Drs. Charles Daley, Jared Eddy, Gwen Huitt, Shannon Kasperbauer)
All fellows looking to rotate at National Jewish Health must have a state license on file.
The National Jewish Health rotation is available to all Pulmonary-Critical Care fellows, however fellows will rotate based on seniority. Typically, this elective is for PGY6 fellows.

Outside Elective Rotation
Our division offers both external rotations to outside residents and observerships to medical students. All requests for an external rotation must be submitted at least 4 months in advance from the requested date and must be at least 4 weeks in duration. Please note, we do not accept ANY external rotations in July.
In regards to observerships, applications should be submitted approximately 3 months in advance from the requested date and must be at least 2 weeks (no longer than 4 weeks) in duration.
